Go through a lifting consultation and you will come across the names of the newest devices. An instrument that has been in use for a long time will have plenty of material behind it, but one released relatively recently often feels unfamiliar. Xerf (XERF), available at our clinic near Gangnam Station, is one of those. Today we will set out what kind of device it is and how far it has been verified, on the basis of publicly available material.


What kind of device is Xerf?

Xerf (XERF) is a dual monopolar radiofrequency (RF) device using two frequencies. It is a medical device made by Cynosure Lutronic, formed through the integration of Cynosure in the United States and Lutronic in Korea. First released in May 2024, it has now completed regulatory approval in 18 countries including Korea, the United States, Brazil, Thailand and the UAE, and obtained European Union Medical Device Regulation (CE MDR) certification in July 2026. Radiofrequency lifting works by passing current through skin tissue and using the resulting heat to prompt contraction and renewal of collagen fibres. Xerf uses two frequencies together, 2MHz and 6.78MHz. Because the depth energy reaches differs by frequency, the delivery depth can be adjusted according to the purpose of the treatment and the area being treated.

What does CE MDR certification mean?

CE MDR is the regulation the European Union applies to medical devices, and this certification is required in order to sell in the European market. Because clinical evidence and surveillance requirements are more demanding than under the previous regulation, whether a device has passed serves as one point of reference. Xerf obtained this certification in July 2026. Approval and certification mean that a device has met the regulatory requirements of the country concerned, of course, which is a different matter from guaranteeing the size of any effect. It is best regarded as official information showing that the device has passed the relevant regulatory process.

What is the difference between the frequencies?

The two frequencies differ not in intensity but in the depth they reach.

6.78MHz acts on the upper and middle dermis, taking on surface tightening and refinement of the skin surface. 2MHz penetrates as far as the boundary of the deeper subcutaneous fat layer, working to contract stretched supporting structures. So where resilience remains but the contour alone has descended, the weighting shifts towards 2MHz, while 6.78MHz can be used where skin texture and fine lines are the main concern.

Planning with Xerf is therefore not a matter of choosing one of the two frequencies but of deciding how much weight to place on each.

What tips are available?

In August 2026, two small tips for narrow and contoured areas were added (XERF i05 and i10). There are places on the face that a wide tip cannot cover evenly — around the eyes, beside the nose and around the mouth, where the area is small and the surface curves. With the addition of small tips, the range over which these areas can be approached separately has widened.

How is it used at our clinic?

The amount of cheek fat, skin thickness and the degree to which supporting tissue has descended all differ from person to person. Proceeding on a set shot count without assessing facial structure can leave one layer excessive and another insufficient. So what makes the real difference is less the specification of the device than whether it is used appropriately for each layer of skin.

Q. Is the treatment hot or painful?

A. Cooling gas is sprayed immediately before and after each shot to lower the temperature of the epidermis, so discomfort tends to be limited. How much is felt varies from person to person, and adjustments can be made as we go.
